“My-MEDIC-Notes™" is a Health Emergency Advance Planning™ tool. The information you record in your personal My-MEDIC-Notes™ lets first responders know during an emergency about your vital medical information and your personal medical wishes. It will also provide hospital personnel with essential details that will help them to provide appropriate medical treatment according to your medical wishes. In a stressful situation, it is difficult to think clearly, and My-MEDIC-Notes™ can provide others with valuable background information about you.

Who is it For?
This program will be of interest to people of all ages. While everyone can benefit from Health Emergency Advance Planning™, some groups are at greater risk .
For parents - you can use it to let caregivers and teachers know what is the first response protocol to help your child through their medical emergency. The medical forms you store in it brief emergency responders and hospital personnel on details concerning your child's medical background. You can use it to create an Emergency Response Action Plan for specific conditions such as Epilepsy, Diabetes, or Anaphylactic Reactions.
For university students - when you are living on your own away from home it is important to document serious allergies and other vital medical information for others to know if a medical emergency occurs.
If you live alone - it can be your "voice" when you cannot communicate your medical background or your medical wishes to others. Many women today are living on their own - divorce rates are up, women live longer than men ... planning for medical emergencies is a must.
